Why Obamacare Matters: H1N1 Edition
There's an H1N1 flu outbreak in Northern California, my college-aged daughter is sick, and the ACA is already making a difference.

When I saw the stories about the H1N1 outbreak in Northern California, I texted my daughter at college in Santa Cruz and told her to get herself to a clinic and get a flu shot immediately.

This is no ordinary flu outbreak, I explained. It's killing otherwise healthy 23-year olds and seems to be targeting young people. Worst of all, two deaths in Santa Cruz have been attributed to the virus. I didn't tell her the stories were scaring the living daylights out of me, but I think she got the message.

Still, I was not prepared for her text to me today, where she remarked on the irony that I would be bugging her to get a flu shot yesterday and today she has a fever and cough.

The cold chills that ran up my spine were paralyzing. I'm 350 miles away from her, I have absolutely no idea whether her fever is just in the low-grade range or worse because she doesn't have a thermometer and just slammed back some Tylenol before heading out to class. Not that fever is a sole indicator, but if it's over 100 degrees it's more likely to be the flu than a common cold.

It's tough to make a mom diagnosis from a distance, and tougher still to communicate a sense of urgency without instilling panic. My biggest concern is that she get to a doctor and get some Tamiflu if possible, or at least just get evaluated to see if it's a cold or the flu. Ordinarily I'd just wait it out, but this pH1N1 flu has my radar on overdrive.

Obamacare to the rescue! Just in the nick of time, we finally got our member numbers on the new ACA policy and temporary ID cards today. I went to the doctor lookup on our insurer's website and was able to find an in-network Urgent Care clinic close to the school. They do have a health clinic at the school, but past experience suggests a much longer wait than I'm comfortable with in these circumstances. I sent her the name, address and phone number and ordered her in my Mom Voice to go to the clinic as soon as possible, especially if her fever rose.

If this had happened last year, we would have been trying to figure out whether she should just go to the emergency room as a precaution and figure out how to pay the inevitable Big Bill later. Now she can walk into an urgent care clinic, plunk down $15 for the visit, and hopefully get some Tamiflu by tomorrow, since the sooner one uses that, the better.

Or, we'll discover that I overreacted to a common cold, she'll get her free flu shot, and we'll all rest better at night.

Either way, I feel much better knowing that she has a place she can be seen immediately for evaluation by qualified medical personnel without having to sign a mortgage on the house for an ER visit.


Here are my takeaways from today's episode of "Obamacare to the Rescue":

- If you haven't signed up for health insurance yet, do it NOW, especially if you're young and healthy. This flu likes young and healthy people. Be prepared.

- Get a flu shot NOW. There's plenty of vaccine available. Do it before you come down with something.

- Do an advance lookup of urgent care providers in your area to have on hand if you have a situation that isn't immediately life-threatening but needs attention. By having the information on hand, you will have the peace of mind of knowing you can get care for situations where your primary provider isn't available.

- Check your medicine cabinet and make sure you have a working thermometer and the basics for symptoms, like ibuprofen or acetaminophen for fever control.

- Wash your hands!

The flu shots are now ***free*** in our state.

Correction: Some hospitals and health department clinics are now offering free flu shots in our state.

Only one fatal case of H5N1 so far in the US, a woman returning from China where it is starting to become of concern -

While this year's vaccine covers the primary strain of swine flu (H1N1), it does not cover bird flu (H5N1)


Again, keep those hands washed, know where your urgent care places are located, and still best to get the vaccine even if not free or you have no insurance. If you get the vaccine and still come down with the flu, it's likely just one of the myriad of other upper respiratory aliments not covered by the vaccine but don't take any chances particularly if the H5N1 becomes more prominent in the next few months.

Interesting that if the ratio of healthy-to-not-healthy sign-ups were locked in today, the 2015 premiums would stay about the same. That should mean as the ratio goes up in the coming months, there's a good chance 2015 premiums could go down. That will add to the two other factors of (1) the MLS ratio leading to rebates and (2) more insurers coming in to compete now with that the bugs have been ironed out; all three pointing to perhaps rate reductions in 2015. The icing on the cake would be if Congress could get into a habit of raising the subsidies just a tad each year similar to SS.

Also interesting to note the prevalence of Silver plans at 60%, three times that of the Bronze plans at 20%. Bronze plans only represent 1/5 of the market. Perhaps in making the case for costs of deductables/co-pays, the Bronze plans should be considered the exception and not the general rule. I realize that takes away from some of the croaking witnessed here, but it is what it is.

Finally, the less than 1% opting for the catastrophic plans. These were originally offered to just those under 30 years of age, but they are now offered to anyone who 'lost' a pre-ACA policy. Lots of hysterical croaking reported over people losing these cheapo 'insurance' plans, but not many takers and certainly not much reporting on it. Wonder why? This may go up in percent, however, with the expected increase in signing up invincible as we close in on the March deadline; although if the Swine flu (or God help us,the Bird flu) begins to become more reported as hitting the 20-somethings hard, we may get a rush on Platinum plans!

Yea, H7 seems to be the one getting started up again in China, which makes it strange (and a little scary) that it was the more evil twin, H5, that caused the lethality in California - H5's lethalness is obviously expected, but where the hell did the evil twin come from??? I thought for sure it was going to be proved a mistyping but when CDC went in with the big guns to specifically prove that, they instead validated the type. Not much press for obvious reasons but if they get a second case...

You can't play whack-a-mole with bird/pig flu strains and expect to get them all when the animals are crowded together by the millions. Especially if the infected animals are asymptomatic!
It makes the idea of flu vaccinations seem trite.
We're on the same page on this; at least the problem. Putting aside the animal ethical issue, it's another example of mono culture being an affront to mother nature - and mother nature doesn't like to be F'd with. If H5's asymptomatic in pigs transfers over to a significant number of humans, we are F'd; it would make Typhoid Mary a footnote in pandemics history.

Just not sure how you get large modern societies to give up mono cultures; it would make global warming debates look like kumbaya singalongs. Maybe the health card and the animal ethics card will get us there, but I'm afraid it might take loosing a staple (my bet is bananas) or a pandemic with farm animal origination. Of course, for the latter one, commercial ag is going to point to it being from small peasant farms in China and that their mono culture approach is too regimented to let that happen. Yea right. :

Its interesting that I'm reading this right now. I just got back from getting "poked". Apparently vaccines are getting hard to find around here, but I was already on a waiting list. I'm just glad to get it over with. There is some debate about the pros and cons about flu-shots in my household, but at the very least I thought it was wise for me to "take one for the team" sorta speak.

Yea, but them thar bovine operations, as you know more than myself, are filled with idiots and a-holes, e.g., putting ground-up sick animal meat into the feed, and getting Mad Cow disease. I don't trust any of the system at all. I'm not quite a vegetarian, but I'm sure close to being one for this reason alone; as you also know, it's a hell of a lot easier if you love S. Asian cuisine!

I liked the scene at the very end of "Contagion" that showed the bat infecting the pig transmitting to the cook's hands who infected the Western lady who then infected the world (except, of course, her husband, Matt Damon!). However, pretty preposterous or at least misdirection of it coming from essentially a one-step- above-subsidence animal raising rather than from some huge animal factory. For it to come from a single family operation the odds must be a gad-zillion-to-one; churning out 1000 pigs a week from a 10K+ animal operation, those odds come down considerably. It's all about the money, and most people just not wanting to stop and consider.

Check out the following book:The Great Influenza: The Story of the Deadliest Pandemic in History by John M. Barry

It's a first rate story and he goes into some of the technical aspects of the influenza virus, and very likely describes much of what will likely happen to us again.

One can't help but wonder why we leave the design and production of vaccines to the pharm companies. As for-profit orgs, it's not in their best interests to design and produce them. Probably why we invariably have shortages, etc. Can you imagine the CF if a novel flu virus really got going? Of course, it would be months before a vaccine could be available.

We know that millions of uninsured are being moved to Medicaid and will not pay anything - do you really think no one under 30 is making less than the $15K to qualify? We know that over 6 million will be paying less than $100 a month for real insurance under the exchanges and a majority of those will pay nothing or less than $50 a month - again do you really believe that no Millies are in the income groups that will benefit from that? You really believe only old Xers and Boomers have low incomes??? What fucking planet do you live on???

If you don't have any, you know, actual FACTS, to counter these very basic facts, then what you are doing is propagating horseshit about something that has nothing to do with you because you have sufficient income and/or employer-based, heavily-subsidized insurance. In other words, you are an elitist asshole.

One upside would be simplicity, said Laura Martin of the Progressive Leadership Alliance of Nevada. Some doctors spend as much as a third of their overhead dealing with insurance documents. It would also make life easier for consumers, who would no longer have to pore through reams of paperwork to figure out which insurer covers what, according to the Las Vegas Review Journal.

It would also make life easier for consumers, who would no longer have to pore through reams of paperwork to figure out which insurer covers what, according to the Las Vegas Review Journal.
I hope that several states will try this on thir own without another mandate . Then we can see how it works . If this woks as well as many expect, then the other states could follow with thir own plans.

From 'I don't want any part of Obamacare' to 'It's a godsend'

....Last year, TIME published a massive special report, "Bitter Pill: Why Medical Bills Are Killing Us," detailing just about everything that is wrong with the nation's health care system. Central to that story were Stephanie and Sean Recchi, an Ohio couple with two kids who had just started up a new business, and who had just been struck by Sean's aggressive and expensive cancer. The author of that story, Steven Brill, has a an update to their story in the current issue of TIME.

Sean Recchi's now in remission, but only after the underinsured family (their $469/month policy was worthless at the MD Anderson Cancer Center in Houston) borrowed from family and maxed out their credit cards. Here's Stephanie Ricci last October:

I don't think Obamacare will help us. I don't want anything to do with it," Stephanie Recchi told me a week after the launch of HealthCare.gov on Oct. 1. "I hear a lot of bad things about it—that it doesn't cover pre-existing conditions and it's too expensive," she added, referring to what she said were "television ads and some politicians talking on the news. Just a lot of talk that this is a bad law."

Did I mention she's an Obama hater? Nonetheless, she tried and tried again to navigate HealthCare.gov, to no avail. That made her hate the whole idea of Obamacare even more, but she needed health insurance, and so went to her insurance agent.

"When they came to my office, Stephanie told me right up front, 'I don't want any part of Obamacare,' " recalls health-insurance agent Barry Cohen. "These were clearly people who don't like the President. So I kind of let that slide and just asked them for basic information and told them we would go on the Ohio exchange"—which is actually the Ohio section of the federal Obamacare exchange—"and show them what's available."

The upshot? Because they're still in basically start-up mode in their business, their income for the family of four qualifies them for expanded Medicaid. If a big contract they are anticipating comes through for them this year, they'll be bumped off Medicaid, but will still qualify for a subsidy that will put their monthly premiums at $566.
What Stephanie soon discovered, she told me in mid-November, "was a godsend." [...]
"Here I get full protection for $566, compared to no protection for almost $500," Stephanie says, referring to her old plan that had cost $469 monthly and that MD Anderson had scoffed at. "This is wonderful. [...]
No, we don't get MD Anderson, but we do get the Cleveland Clinic and lots of other good care," Stephanie says. "We understand that."
